Makati High School School Clinic Student Services Flow
Return back to school of learner who had Communicable disease (e.g. Chicken pox, HFMID, Measles, Mumps, Pulmonary Tuberculosis etc.)
Phase 1: Entry & Assessment
The student enters the clinic and must present His/Her medical certificate.
The nurse will check the medical certificate and Assess the student for fitness to attend class.
Determine: Is the student cleared to attend class?
Phase 2: Clearance Granted (YES)
The student will immediately proceed to His/Her classroom.
Phase 3: Clearance Denied (NO)
Inform the adviser and parent that the student must be sent home or referred for further care.
Give the student a discharge slip and referral form.
The parent/guardian will sign the slip to acknowledge the decision.
Determine the next step: Is the student to be sent home or for referral?

*Students needing a scheduled appointment must pick up a Parent Consent form from the Dental Clinic.
